club rules and you undertake to reimburse the club any expenditure incurred on your behalf in respect of race entry fees, club kit or other costs, and to pay any subscription fees promptly as they become due.
By checking this box, you agree to notify the Club Welfare Officer of any condition (or change in personal health/ability to swim) that might put you, or others, at risk. In particular please tell us if you suffer from any of: diabetes, epilepsy, blackouts, asthma, bronchitis, ear problems, muscular/skeletal problems (e.g. back pain).
By checking this box you understand that in order to partake in sculling activities you must have successfully completed a Capsize Drill approved by the Club, and you agree to sign and return a Champs sculling disclaimer form.
By checking this box you understand that some information about you such as your name and email address may be stored by boat organisers on their own computers and in shared on-line documents for the purposes of scheduling outings etc. but that it will not be shared outside the club without your permission. You also understand that we will add you to a club-wide mailing list (which you can then opt out of if you wish).
